The form gets populated with some read-only information (see URL below).

The form used to work fine, but I had to change the Google Sheets formula that creates the URL because we had names with spaces and hyphens.
The formula now uses EncodeURL instead of just concatenating the information together.
GSheet formula -> ="https://www.jotform.com/52035470710142?"&encodeurl(I2)&encodeurl(J2)&encodeurl(K2)&encodeurl(L2)&encodeurl(M2)

I have checked your form and you have set the fields to read-only.
To clarify, do you want to populate the fields using URL parameters?
Unfortunately, the URL you have shared will not work.

I set two key fields to read-only and they are the ones that are passed via the URL. It looks like the ENCODE() doesn't work. I edited the data to remove any spaces in last names.
I wonder if converting the spaces to be %20D in the URL would have worked?
One additional question. Why does the checkbox item "I Agree" appear on two lines?

We cloned your Form https://www.jotform.com/build/90905713393965, and did a preview mode and saw that the I agree option was coming up correctly on the form.
