Login autosuspended

    I'm trying to create a new form by copying an form I just created in Wufoo. It appears to upload then a page pops up for me to login to JotForm. I enter my credentials then I receive "Login failed - AUTOSUSPENDED." I've tried resetting my password and I continue to recieve this message. What do I need to do to proceed?

    The form http://www.jotform.us/form/90894566727171 was found on your account as suspicious of phishing activity because you're collecting user credentials.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
