How can I upload forms for referring physicians to fill out and sign?

    Hi,

    We are a radiology company that have forms that we need our referring physicians to fill out and sign (need to be HIPAA compliant). We used DocuSign before and would like to use something similar where we can have the physicians and staff electronically fill out and sign these forms. How does JotForm work and is there a free trial so that we can try it out for a bit?

    First of all, we don't offer a trial period. You can use all the features of JotForm with a free account. But just the HIPAA compliance is only available for silver and gold plans. Please see our pricing page.
    You can use the Adobe Sign widget to get signatures electronically. This signature method has legal validity.
    Also, physicians and staff can draw their signatures with E-Signature and Smooth Signature widgets.
