Hipaa form with file upload delivered as pdf

Hei, I try to make a HIPAA form with information and also file upload.

I want to receive this as a pdf.

When I open the pdf I have the correct, but the link to the File upload leads me to a 404 page.

From the inside of the PDF editor the File Upload link actually works. But not on the final pdf I receive in my email notification.

That may be happening if you're logged out of your account, the HIPAA accounts require you to be logged into your account to access the files, please try this and let us know how it goes.
