How can I receive emails with complete forms?

I receive emails with my forms but I only receive the data and not the text on the form. Is there a way to receive the form with the data?

I have inspected your Alba Wellness Intake form. It has five separate text fields. And you aren't able to insert them into your notification email. You can insert text by typing here(or pasting).

If you care layout of text(for example first and last fields), you can use their source code to insert. Please look at these.
1. Copy your text field's source code.
2. Paste into notification email source code. 
You can see all the email content in the source code. If you don't know where to paste, I recommend that you paste it behind the </table> label.
