Trouble with form submission

Someone just tried to register for the event I created and could not pay for the event.  I would really appreciate some assistance.

Please note that the form is being submitted once a Submit button is clicked.

A Submit button seems to be currently missing in your form. Please add it to the form in order to fix the issue.
