Form is not allowing movies to be uploaded

When attempting to upload videos to our Video Award form, it says you cannot because the extensions aren't correct (something to that effect). However all the correct extensions at accurately listed in the question so I don't know what to do.

Please omit the dot symbol from extensions. It should fix the issue.
