Bookkeepers need to know where to send the check.
I put that information in the header, but it is not appearing
on the output form. Is there a way to pass that payment
information to the bookkeeper emails?

Text elements are not included on emails because they don't ask user value. What you can do is to copy the text from your form and paste it in your form notification or autoresponder.
