Users are getting "this page isn't working"error message after submitting the form

    After a quiz is taken, users are getting this error:

    Can you tell us the form URL to which form this issue is related to? Also, are you facing this issue on the standalone form or on the webpage you have embedded?

    Could you please provide the webpage URL you have embedded the form if this issue encounters on your webpage?
