Why did I not receive form submission come on this is ridiculous

why did submission not come through on "Kickoff Call Form - Financial Cients Fast" geeez come on. this should not be so difficult. 

We apologize for the inconvenience. That form has not received any submission yet. Have you attempted to submit the form? Did you receive an error message? Can we try doing a real submission in your form in order to reproduce the issue that you are experiencing?

No it’s not reproduceable and now the person is pissed he took over an hour
...

I tested your form and submitted without any problems. Not sure why the person filling had any trouble.

Also, while checking your form, I saw this field:

It is highly against our Terms of use to collect passwords so that field has been removed, and please avoid putting it back or your account will be suspended.
