When viewing Jotform today, our address was missing and now submissions are missing with nothing in the trash. We are a government entity and need to view these. Is there a way of restoring this?

I am sorry for the trouble caused to you. I did check your account and found that you only have 3 forms with no submissions.
Do you have any other account? Can you please let us know the form ID, form URL or webpage URL where you have embedded your form and received submissions? Do get back to us on this and we will take a look.

I'm not sure what seems to be the issue.
Your submissions page for that form is https://www.jotform.com/submissions/31795010277150
And that is working fine, but the form is encrypted, so you have to decrypt the data in order to view.
If you're still experiencing the problem please give us more information about the issue.

LLWA20Replied on April 3, 2019 at 3:43 PMWe are still experiencing the issue. We are decrypting the submissions as we can see everything clearly. The issue is that several submissions disappeared, and the mailing address (which is required) is a part of the created form, but is not a part of the form when submitted to us. It is lost between the applicant completing and our receiving. Please advise.
I am not sure what additional information to provide.

Submissions have not disappeared, the account logs show that you have been deleting them:
https://www.jotform.com/help/370-How-to-View-Your-Account-Activity-Logs
Unfortunately, once a submission is deleted, it can not be recovered. Let us know if you have more questions.

I have sent a test submission in your form. Can you please check your form's submission page and see if the address field is being displayed correct. Please ensure that the Address field is selected in the list of form field. Please check the screenshot below on how to select questions in form's submission page:
