When I receive an email that someone has responded, it comes with a PDF of their "application" attached.
While everything looks great on the application itself, AND their responses in the body of the email, the PDF version has the questions all out of order.
Attached is page 1 of the PDF (since it will only allow me to upload a jpg). But you can see that some of the questions are being inserted into the text sections.

We are sorry for the inconvenience this may have caused.
Can you please try creating a new PDF document and use that?
To create a new PDF document, click on "New PDF" then New PDF document.
And add it to your notification email.
Please give it a try and let us know if the issue still remains.
