Can form questions be embedded in an email?

I am looking for a way that the form fields can be in the body of an email and be active.

Ie. question with Yes or No single choice checkbox in body of email. I would want the response to result in an action (send a response email, allow new question to pop-up).

Unfortunately, it's not possible to embed a form in emails, most of the email providers will not allow the code used to get the forms working.

Instead, you may provide the users with the link to your form.

Ok thank you for the response. so when I am seeing this in emails I have received from companies is it that the email provider itself has the functionality? I have seen functional buttons in an email is cirrusinsight.

Yes, they may have customized the emails with the desired content, but they should be using only allowed codes, for example, CSS code is allowed to style the emails.

You may consult with your email provider if you're allowed to embed the form or add buttons. From JotForm, you can only get the form's embed code.
