Text showing up in fields

I have a form,which when I view form, the fields are populated with text and some of the radio buttons are pre checked, which I can't uncheck.

I have sales reps at a trade show and need to resolve this ASAP.

Please advise.

Continue Forms Later is enabled in your form so it remembers the unsubmitted data, saved in your browser, until you submit the form.

Sorry, you can clear your browser's cache and cookies if you want to delete the prepopulated saved data or you can disable the option I've mentioned.
