Database Widget Available?

I would like a widget or a way to select/enter an employee's name and the value of points populates.  Is there a widget or a way to do this?

For example:  I type Mike and in another field, 400 would be connected to Mike's name.

The closest way we have is spreadsheet to form widget: https://widgets.jotform.com/widget/spreadsheet_to_form

In that case, you would have to have a sheet prepared with such data, so when "Mike" is entered, a "400" would be inserted in a field.
