Unfortunately, it is not possible to create login forms or ask for any sensitive information such as passwords, etc. Such forms are considered as phishing forms and the associated accounts are automatically suspended. Please refer to our terms of usage for more information on phishing forms.
However, you may set conditions on the form to show/hide fields based on a field entry that can be a verification code. Please refer to the guide below:
https://www.jotform.com/help/196-Hide-or-Show-Multiple-Fields-at-Once
Thanks!

Hi @Luca!
You can clone the following form to see how the verification code works: https://form.jotform.com/90543660719966
Here is a quick guide on how to clone forms: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
Then, simply go to Settings>Conditions to be able to see the Show/Hide Field conditions that show or hide fields based on the input in the Verification Codes field. You can click on the blue pen to edit each condition and pick any numbers to be your verification code.
After you are finished creating your form, go to Publish where you will be able to see the URL of your form. Simply share this link with people you want to complete your form, along with the verification code via email.
Please let me know if you have any other questions. Thank you and have a great day!
