My account is suspended

    Can you please re-enable my account? I am guessing it is suspended because I was collecting an SSN as part of an application form for my apartments. If my account can be restored I can add encryption and the necessary consent to the form to bring it up to compliance.

    This form http://www.jotform.us/form/80596055255158 was found on your account as suspicious of phishing activity because you're collecting social security and driver's license numbers.

    Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.


    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things:


    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.


    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.

    Yes, I agree to this. I'm collecting data for my customers (tenants).

    The infringing fields have been deleted and your account has been reactivated.

    You can add the identification numbers and add the mandatory widgets in order to avoid further inconveniences.
