User not anle to submit the form multiple times.

una cliente non riesce a compilare il modulo di iscrizione con forfora perché dice che può mettere una sola voce e no voci multiple, ho allegato lo screen e non ci sono scelte multiple... ho allegato anche il link dell'avviso che esce.. potete indicarmi per favore come risolvere?

I see that Unique Submission field set to "Check cookies only". This will restrict multiple submissions. If you wish to disable this option, please go to Settings > Form Settings > Show More Options and set the Unique Submission field to No Check.
