Why do some of my new patient forms have a picture of a map?

    It looks like Jotform has changed a few things and I like the changes but before we had an option, when printing out new patient forms, to have a diagram of a map of the office location. We don't need that and I knew how to get around it before but today I am seeing the new patients we have in the que  with the diagram. Is it a setting I may have clicked unknowingly?

    In the submission view page there is an option to disable the map included with an address:

    Unchecking that option should remove the address map.
