How can have a user add another row to an order?

For instance if someone wants to order 2 of the hoodies I have shown, one in a Small and one in a Medium. Is there a way to add another row to complete this? I am using a template that was available but does not give me what I need. 

I tried to create form from balnk and have tried the configurable lists which I can successfully add another row but when I have multiple orders like a Hat and Hoodie it does not come through on excel sheet.

Please advise to get either of these to work.
