Can't get PDF attachment to work

On the autoresponder to my What to Wear and How to Prepare request form, I have the autoresponder set with a PDF attachment.  When I send a test, there is no attachment.

The test email would not contain submission PDF's or attachments.  If you want to test those, it would require sending a submission through the form.  The emails triggered from the submission should have any files attached.
