    1) Some of the fields worked,

    2) others have duplicate information from other fields and

    3) two are blank.

    Here is a screenshot:

Can you please check if the right fields are set on your clinchpad correctly? See the field names and see if that matches the fields on your form.

Also, try re-integrating the form to Clinchpad and see if the issue persists.

Clinchpad says to just use the JotForm field label as the text you put in their fields.

Can you send us a screenshot of the mapped fields in Clinchpad? We will clone your form and integrate it in Clinchpad to test it on our end.

Please show all the fields and label names in the screenshot.

Start by click on "Settings" at the top right of the screen. Then make sure you use Custom Fields for "Leads" not for "Contacts."

I just realized that you need the payment field deleted to work with the form, so I did that. It works without the payment field now.

Sorry for the delayed response. I think the problem is in the labels of your field, field labels should be unique. The first part of the label should be not the same and no special characters.

I got my clone form to work properly, check out my demo form here: https://form.jotform.com/90955231531959 and change the labels of your field.

Please give it a try and let us know how it goes.

It worked! Kudos to you, jherwin. I will let Clinchpad know, because they were trying to solve it for me, too.

It is weird that one multiple choice question put the 3 responses into 1 line, and another multiple choice question put the 3 responses into 3 lines. See below:

Again, thanks for your help. I am amazed that you figured out about the field names not being similar in wording or using special characters. I would have never figured that out. Really appreciate your digging in on this, JHerwin!

There is a little difference between two multiple choice fields. "Areas in which your student has difficulties." field uses calculation values. However, "Check all supports your child currently has." does not use this feature. Some of my JotForm data is not flowing into Clinchpad accurately. Image 10Some of my JotForm data is not flowing into Clinchpad accurately. Image 21This option changes the value that fields return. Can you please disable this feature for "Areas in which your student has difficulties." field to see results in one line?
