Old title of a cloned form still showing up in the tab? How do I get rid of it?

Hello,

I cloned a form that I am using for a program but the dates are different now. I've changed the dates in every spot that I am aware of but when you open the form in the safari tab it still shows the title of the program with the old dates in tab but in the form the dates are correct?

Any help is appreciated!!
