    Hello,

    Yesterday I emailed to have my account reactivated as there is no phishing going on. It is a horse registry site with 2 forms. One for registering horse, the other authorities. Could YOU PLEASE reactivate it ASAP. I have heard nothing back and it has been over 24 hours.

    The form http://www.jotform.us/form/90935288191163 was found on your account as suspicious of phishing activity because you're collecting user credentials.

    Gathering this type of data violates our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.



    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.

    Yes I will take the field out all together but my account is suspended so I can't do it!
