Auto Suspended

Extremely frustrated that accounts for both of my businesses accounts are being auto suspended in the middled of working on projects! Please help me rectify this asap.

The form http://www.jotform.ca/form/90941055442252 was found on your account as suspicious of phishing activity because you're collecting user credentials.

Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. We are running a photographers virtual assistant company so we are required to have credentials for the work our clients are hiring us to do . A form like this is the easiest way for to save and access and access code would be confusing to some of our clients
