Why is my form showing up as a lot of code in my email?

when I submit my form it shows up in my email as loads of lines of code. There should be a picture and a description. 

That is because you are copying the data value of the Take Photo Widget to the Calculation widget. I don't why you are doing this, but this will not work in the form. There is no option, as far as I could see, to get the file name (if that's your purpose).  You have to delete the calculation or remove the widget in the calculation to fix this.
