Add a form to my account

Hello I created to 2 forms on your site before i signed up, when I setup my account only one of my forms are listed ( partner with us) is the title, how can I add the (become a driver) form that I designed on your site also?

I have checked your account and I can see that there are 2 forms on your account and both of them have the same title "Partner with us". I have also checked the Page URL and when I click Become a Driver option, it loads the same "Partner with us" form.

Have you created the other form using some different username or account? if yes then can you please provide us that username or form URL/number so that we can assist you accordingly.

Thank you for your information, I see the mistake I made and I will create a new Become a Driver form.
