How to create a form with multiple dates

Good afternoon

I would like to create a training registration form for a free mandatory in office training. 

I don't see any form like this .

The form would have multiple dates to choose from.

I have created a form that lets you pick 3 different dates from the drop-down menu and also has an option to select event dates based on the number of slots available.
