Suspended

    I keep getting a message that maybe my account was suspended.  I forget the reason (something with my IP address) It says form not found, form disabled, but I think it may be because I was on VPN.  Can you activate my account again please?

    These forms were found on your account and flagged as copyright logo scam.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #1786599 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the logos and lift the suspension on your account.

    For now, please remove our logo. I will work on the other items at a later date to add our logo back to the forms. Thanks.
