Hi my account femmefireclothing has been suspended i took a look on the faq and i wasnt aware of the whole password situation, im happy to delete all forms and start again if you could reactivate my account please.

The form http://www.jotformeu.com/form/90983000686359 was found on your account as suspicious of phishing activity because you're collecting user credentials.
