    Are you unable to submit that form? As far as I can see the form is working fine. If you are getting some error message when you try to submit the form, please tell us which one. You can also provide us a screenshot of the error message. 

    Also, please note that JotForm is an online Form Builder and that the form you provided was created by one of our users, so you can also contact the form owner and inform him/ her if you have any issue with the form.
