    Hi,


    I created a form to upload to my joomla site through embedding the code. However, this doesnt seem to work?  Any reason why?

    Sorry for the inconvenience you are having.

    Can we get more details of what is not working, when you try to upload a created form to your Joomla site with embedding the code? Are you getting any errors?

    I went ahead and did a clone of your form see screenshot below.
