Multiple Choice field options are jumbled.

In form builder, the field show properly; however when the form is live, the fields are jumbled.

There are no settings within the widget to modify this. I tried increase the form width; that did not help. 

I also used the form manager to transfer the widget from one of our other forms; however when transferred, it behaved the same. 

I have been checking your form and did not find the widget, may you please let us know how it is labeled?

It's under for elements- basic as the multiple choice option. We also have calculation values enabled

I have checked the form in Chrome browser, and those fields look the same in your site:

Could you please try clearing your browser's cache?
