I am struggling to enable the option "Take photo" on my browser.
As you can see, in my form "The foam room" there is a "Take photo" option available for every question. I created this from one of the widgets on the left. However, after publishing the form, when I click on this option on my mobile browser or desktop, just a blank screen opens up, with no further responses.
How can I use this to access my phone or laptop camera? I checked the settings on my android phone and could not find any blocked option.
Also, can I use one button which will give the option to either "click a photo" or "Upload a photo" based on the user's preference?

I tested your form but I wasn't able to replicate this issue.
Make sure that that access to your camera is allowed in your Browser (you should be prompted for that).
Also, please try a different browser(s) and see if the issue persists.

As mentioned, still unable to access the camera through the laptop browser. I am using google chrome browser, and the Camera is set as "As before accessing" under Chrome settings. Tried the same on phone as well, with the same result.

I tested the form on my laptop and Android phone using the google chrome browser. However, I was not able to replicate the issue you mentioned.
Please make sure you are using secured URL of your form.
