Is there an option for a drop-down menu with jot forms.  Is there also an option to answer one question and it takes you to the next question if the answer is "yes".  If the answer is "no" it takes you to questions 5 (for example)

I would suggest you to please take a look at our "Dynamic Dropdowns" widget and see if that fits into your requirement.

Yes it is possible to achieve your requirement by taking advantage of our "Conditions" feature. You need to add condition to check the answer of a question and then show another field. You may like to take a look at the following guide which should help you:  https://www.jotform.com/help/57-Conditional-Logic-for-Online-Forms

    Hope this helps.
