Why aren't certain fields retaining the data?

Two fields in one row field is not populating as it mentioned in the forum.  Second row comes, it leaves the left field empty...can't populate...Any suggestion to resolve this issue...I am using chrome...is it a browser issue or Form issue?

Is it on emails or are you filling the fields via the form URL?
