My form looks fine and so does the pdf, but when someone fills it out and I download the pdf it is all out of order and missing parts

    My form looks fine and so does the pdf, but when someone fills it out and I download the pdf it is all out of order and missing parts. the screenshot below shows how everything is out of place and not how the form actually looks

    We apologize for the inconvenience caused. There is a glitch in the default PDF document template of the form if you did not use the PDF editor to create it.

    To fix, just reset the PDF template by creating a new PDF Document. Click on the +New PDF button...

    The new PDF document should be formatted properly then.
