My Multiple Choices field is not displaying correctly

The formatting issue with single/multiple choice fields should now be resolved.  Let us know if the issue persists in your form and we can take another look.

but still have problem on check box

Your screenshot is showing that the checkboxes are not there, only the options. I checked your form, and they're there so maybe it is your browser. Please check with other browsers to see if you can replicate the problem.

and it is not because of the browser..maybe cause of glitch on server..

cause even on android same display..
