Drivers return form

Hi
the appearance on the screen has changed but we have done nothing.
Questions with radio buttons should be over two columns but show as only one.

text is running into the radio buttons etc

Please advise whats happened and fix 

The issue with the Radio Button field was resolved by our developers. Please check it again.
