Form not working in mobile device

    My JotForm is not working over the phone, just the computer how can that be fixed?

    Can you please explain further the issue with your form when viewed in the mobile device?

    May we know if your form is embedded to a web page?

    May we know if there is an error message when your form is submitted?

    It just says that the URL is not found. 

    How would I go about embedded to a web page or removing that?

    It does say that I do have an active status.

    You may embed your form to you web page by following this guide:https://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page

    I have checked your form and I can't seem to replicate  the issue at my end. I'm using safari browser on an iPhone to view your form.

    May we know the mobile device you're using?

    Is there an error message when you try to open or submit your form?
