Form was changed: All of this was fine when I left it last night.

All of a sudden the "Access" field's options are lined up horizontal rather than vertical?  I deleted it and created again but the option fields line up horizontally and throw the rest of the form out of whack.  Any ideas?  This is the Detailed Quote Sheet form.  There are two forms named the same thing but they are both identical and have the same issue.  Same thing with Customer Name and Computer Crusher Recycling fields.  All of the options are now horizontal and run off the page.  I had them vertical without issue before.

 **Update.  The form has been totally destroyed.  Now I cannot get the "Date of Pickup" field to move over to the right under Date Created.  It seems I cannot get any fieds to move the right.  All of this was fine when I left it last night.  The form is Detailed Quote Sheet and it is starred as a favorite.

The form should look just like the URL shows and was like this yesterday when I closed it out.  I did add the time field option to Date of Pickup so I can now delete Time of Pickup.  I still cannot get them to line up in 2 columns as before.

This is still not working.  Currently on form https://www.jotform.com/build/91015653577157

I have Date of Pickup on the right hand side.  Everything looks perfect.  However when I go to print the form Date of Pickup moves to the left under Contact name.  Please help.  I have read the How to Position fields tutorial but everything still goes flying when trying to move fields around.  Please help.  Somehow I got it back to how I wanted.  However why is there such a big space between Date Created and Date of Pickup?

Looks like the fields are currently placed on different lines. The 'Alt Phone Number' is set to be placed on a new line, so there is a gap after the 'Phone Number' field.
