As I check your web page on my Android mobile device and see that the background video is being played normally. However, as I check the web page on an iPhone emulator, I am able to replicate the issue at my end. I have also tried cloning your Jotform and see that the issue still persists after tried changing some settings.

Let me forward the issue to our backend team for further investigation. Once we have any news in this regard, you'll be posted here.

    Thank you!

Hey its been 4 days since I last heard from support and I need to launch soon.
Do you guys have a better image choice/slider element I could use as well? One that looks more professional.

I also came across another issue, some of my elements or widgets are incapable of displaying the title without it overlapping over the border of the form, therefor I have disabled the title for this elements/widgets. I would appreciate having the ability to use the titles for these elements without it looking sloppy and overlapping the border of the form.

A VP8 encoded video is not fully supported by the mobile Safari. You may try an H.264 encoded video instead. The H.264 video will most likely work on all major platforms.
