How can I add an additional line ex. Name . Middle . Last

On the attached form I am only able to use Name last.

Also same thing for phone #

I am not sure if I have understood your question correctly. For guest 1 field in your form, you seems to have added full name field. Do you want to add middle name as well?

Please check the screenshot below on how to show middle name in full name field:

If you meant to add field in a new row, the only possibility is to add a new short text entry field in form.
