A card keeps showing up even though I have every thing hidden based on a selection

in the TCS stack, I have a card that gives the user 5 options to choose from.

Based on that choice I have hidden a series of fields to make sure they only see the fields relevant to their choice.

When I choose anything else than "full stack" this random section header shows up after the "what is your first name?" question field. 

Inside of the conditions where I hide all fields that are supposed to be hidden, this Section header is not showing anywhere.  

Please help me hide this field.

I have checked your form and see that there are a few conditions with missing fields which seems to be causing the issue.

Please review the show/hide conditions on the form again and let us know if you need any further assistance.
