I want to send PDF attachment of form to dedicated email

    Hello


    I would like to subscribe in your good service but first I wand ensure that will accomplish my requirments.


    I want my employees to fill up the form and submitt and delivered it to a dedicated email in PDF as the attached picture.


    This will be used in an Airlines company.

    Hello,
    Firstly, you must re-create form in JotForm. After that, you(or dedicated email address) can receive form submissions as a PDF attachment via notification email. Please see this article.
