How to pass a custom donation amount to Payment Form?

    I need to create a donate form the mirrors the attached. I have used the purchase product template but it will not give me the other entry option which allows form user to input desired amount.

    If you want to enter the custom value then you would have to use the traditional Form elements and then add it up in the Form Calculation widget. Once you have added then pass it to the Payment field in the user-defined selection.
