Automatic communication with requestor and email defined on form

    Hello, We are currently looking for a form creation application with some kind of communication automation  with the requestor and an email defined on the form.  Please find a set of requirements attached to see how your application can meet them.

    The set of requirements as you have mentioned has unfortunately not registered on the support thread. From what you have mentioned, I think you an email to be sent out to the person filling up the form and this email address will be as filled up in the form.

    If this is the case, we have the autoresponder feature where you can automatically send emails to the people who have submitted your forms. When you set up the autoresponder, you can select the email address field from your form in the recipient email dropdown as shown in the screenshot below.

    If this is not the case, please explain your requirements further.

    You can upload it to the dropbox or google drive and share the public link with us so we can download or view it. Also, please provide additional information about the workflow of your requirements so we can check if it's possible or not.

    I have sent you a link via google sharing.  Also here Requirements.  The process flow if very simple:  the person filling out the form will identify an email address on the form for themselves and another for their boss.  Upon submission of the form, emails will be sent to these two individuals.  Form information will be saved somewhere from which an administrator will download the information and put it into an google/excel sheet.

    We do not have permission to view the file you shared. Please make it public. 

    You can add two email fields and Autoresponder Emails in your form and send them to the email addresses entered inside fo the email fields.

    You can also use conditional logic to send emails based on the user's answer.
