Is there a widget that can add multiple answers for one question?

Our form requires conditional answers.  For example, after parents enter their information for guardian#1. I want to ask the conditional question, add another parent?  similarly for other sections, like  add another emergency contact?  add another student? etc.

How do I add conditional options to our forms? we are registering kids for our Summer DAy Camp.  Most families have multiple kids to enroll.  So therefore, we need that option: parents can add 1 child or multiple.

Please advise ASAP.
