Import Jotform App is giving me an error msg when i try to upload, also its not showing all the fields i need to add.

I have checked your form and cloned on my end in order to investigate the issue. However, I was not able to replicate the issue.
Could you provide us with the screenshots of the issue and the error message for further investigation?

I wasn't able to replicate this issue on import app:
Did you modify or alter the excel you're trying to import?
Are you still able to replicate this problem?

No but you can see in the screenshot you have attached that zero submissions were received.

I tried importing the submission in the Import App and it got imported successfully.

Could you please match the CSV file with yours and try once more.
