Recover lost account

    Hello,

    I'm the web developer for http://www.dealerservicesint.com.


    Currently we're using JotForm on a few pages and are getting a "form over quota" on the site. 


    We've gone through some structure changes and have lost access to our account. 


    Can you please assist in recovering our account so we can fix this error?

    You should be able to access tgrachen@dealerservicesint.com in order to recover this account (by clicking on "forgot password")

    Otherwise please send the following billing information for past payments on this account:

    Transaction Date for the last subscription renewal and amount charged by us.

    Credit card last 4 digits and expiration date of CC.
